POSITION TITLE: Caregiver Support Specialist

REPORTS TO: Director, Senior and Caregiver Support Specialist

WORK HOURS: 25 Hours Per Week

WORK LOCATION: MidPointe Cincinnati

Position Summary

The Family Caregiver Support Specialist provides education, support, resources, and assistance for family caregivers with identifying needs and subsequent action planning. The Family Caregiver Support Specialist also recruits, trains and oversees peer mentor volunteers to build capacity for caregiver support services

Essential Functions:

Administrative

  1. Assist with program data management including conducting intakes and entering client information into Salesforce and Council on Aging systems.
  2. Assist with planning and executing program special events and retreats.

Caregiver Support

  1. Connect by phone, video call or in person with family caregivers to develop customized action plans that may include problem solving, self-care, self-advocacy, coping, linking to available community resources and ongoing support to reach established goals.
  2. Conduct Powerful Tools for Caregivers classes, as needed. Training to become a class leader will be provided.

Caregiver Peer Support Program

  1. Recruit and utilize appropriate former caregivers as volunteers, to act as peer mentors.
  2. Provide training and oversight to peer mentor volunteers.

Qualifications and Skills Requirements:

  • Possess patience, good discernment, and discretion;
  • An interest in creating relationships with new people and helping them to connect with services/resources in the community;
  • Thorough knowledge of older adults and of family caregiver responsibilities, stressors, issues and needs;
  • Ability to work effectively with family caregivers from varied socioeconomic backgrounds;
  • Ability to communicate effectively in oral and written forms
  • Ability to present information clearly and concisely when appropriate;
  • Flexibility, organizational and time management skills for independent daily management;
  • Possesses the ability to travel within the following Ohio Counties: Hamilton, Butler, Clermont, Warren, Clinton.
  • A minimum of 1years of experience working with older adults.
  • High School Diploma/GED.

Preferred Qualifications and Skills:

  • Experience working with family caregivers or in a care setting;
  • Bilingual in Spanish; and,
  • Experience utilizing volunteers to build capacity to serve clients.

Work Environment:

Typical office environment, with minimal exposure to excessive noise or adverse environmental issues.

Must be able to meet the following physical requirements, with or without reasonable accommodation:

  • Use hands to manipulate, handle, feel, and control items or equipment
  • Talk to other employees/clients and hear them
  • See and be able to read, write, and interpret written documents
  • Occasionally lift and move objects weighing up to 15 pounds
